Our LifeCare Assessment changes that. In one focused session, we review everything that matters: who to call, where to go, what medications you take, and how to stay safe. Then we put it all in writing so you (and your family) know exactly what to do when life gets complicated.

We ask about your emergency contacts, health-related information you choose to share, preferences, routines, documents, transport, and concerns. We listen first, because the plan should reflect your life, not a generic checklist.

Step 2: Documentation & Review

We gather the information you've shared and organize it. We may ask follow-up questions via email or phone to fill in any gaps. We also review your home for safety considerations.

Step 3: Assessment & Planning

We compile everything into a comprehensive assessment. We identify any gaps or concerns, and we create practical solutions. We organize your important documents and create your action plan.

Step 4: Written Action Plan

You receive a professional, easy-to-read action plan in English. This document includes your emergency contacts, hospital preferences, medication list, important documents, transport plan, home safety notes, and clear next steps.

Step 5: Follow-Up Consultation

We review the plan with you, answer any questions, and make sure you feel confident about everything that's documented. You leave with a clear sense of preparedness.
